Social Workers
PAHCS has three social workers � April Stadtler, Trish Barten and Kristle Chase. The Social Work department is a part of the interdisciplinary team at PAHCS that serves as advocates for patients, residents and the community. We are available to patients of all ages and stages of life in all PAHCS service areas.
Health Care Access: Weather you have insurance that does not provide adequate coverage, no insurance at all or confused by the many Medicare part D plans and Medicare coverage options one of the PAHCS social workers can assist you seeing if there is an alternative to meet your health care needs.
Health Care Directives: PAHCS social workers can assist patients in ensuring that their health care wishes are communicated to their health care team by assisting them in creating a Health Care Directive. A Health Care Directive is a legal document individual�s fill out to designate someone to make health care decisions for them in they event they are unable to and to write out their wishes, beliefs and values for their own health care. If you are interested in completing a Health Care Directive, please call one of the social workers at PAHCS.
If you need to stay in the hospital one of the PAHCS social workers may assist you in your transition to home. Through discharge planning one of the social workers may help you obtain ongoing care and services.
The social workers at PAHCS coordinate all admissions and discharges to Koronis Manor, Hilltop Care Center and swing bed. They assist patients in obtaining the right setting for them depending on patients needs weather it be longer stay at one of the Long Term Care facilities or short term rehab in a swing bed. Once placement has been determined the social worker will be a part of the interdisciplinary team that will assist in coordinating your care. If a transition to another living arrangement is anticipated the social worker will assist in discharge planning and setting up available services and ongoing care needed after you leave.
If you are staying at PAHCS either in the hospital or one of our Long Term Care Facilities the PAHCS social workers assist patients and residents in protecting their patient or resident bill of rights. Patients or residents who are enrolled in Medicare have additional discharge appeal rights when being discharged from the hospital or ending a Medicare covered stay in Long Term Care/Swing Bed; the PAHCS social workers will assist you understanding these rights and assist you in exercising these rights if you wish.
Information and referral: PAHCS social workers have a lot of information on various social needs ranging from mental health & chemical dependency issues to Children and Family services.
